How price are being determine by the various Exchange?

I am wondering how bid/ask prices are being determine exactly in real-time by
Exchange especially those in the interbank spot forex and any sources
such as books/website/discussion forums that can help me to understand.
In addition I also wonder how I am sure whether the price are determined
in a fair and trustable manner. Thanks in advance.
