Sure have. A couple of times. Happened yesterday.
In my experience, it always relates to either a system lockup or a power
outage while GS is running.
Luckily, I keep pretty up-to-date with backing up my Omega files (MyWork,
Server, and Template folders), so I just restore those folders and use
HistoryBank to catch up on any updates.
I learned my backup lesson the hard way, and had to use HistoryBank to get
data in 3-month increments. It's a process...
